COMMUNITY HEALTH WORKER

Location: Burnet Campus, 3440 Burnet Ave., Cincinnati OH 45229

Shift/Schedule: Day Shift/40 hours per week, Monday-Friday 8a-4:30p. No evenings, weekends or holidays; no on‑call.

Required Qualifications

• Candidates must have a high school diploma or equivalent
• 2-5 years of work experience in a related job discipline
• Have a reliable vehicle/valid driver's license

Helpful Information

• Current Community Health Worker (CHW) certification in the state of Ohio is preferred but not required

Job Responsibilities

• Consult with care team members to address complex issues.
• Supports population outreach to identify population(s) at risk and assist with closing population gaps.
