Job summary

Cincinnati Children’s Hospital Medical Center is seeking a Day Shift Social Service professional at the Burnet Campus in Cincinnati, OH. The role involves coordinating care with the care team and supporting patients and families daily.

• Complete required documentation linked to the care plan.
• Provide updates to families regarding patient condition and progress; refer families to appropriate resources.

Schedule

Day Shift/40 hours per week, Monday-Friday 8a-4:30p. No evenings, weekends or holidays; no on-call.

Required Qualifications

• Candidates must have a high school diploma or equivalent
• 2-5 years of work experience in a related job discipline
• Have a reliable vehicle/valid driver's license

Helpful Information

Current Community Health Worker (CHW) certification in the state of Ohio is preferred but not required.
