Responsibilities

• Provide supportive services to nursing staff as needed.
• Assist residents with activities of daily living per care plans.
• Help with lifting, turning, positioning, and transport.
• Coordinate dining room services, meal delivery, and feeding assistance.
• Deliver nutritional supplements and document intake.
• Record weights and report changes to nurse.
• Monitor skin condition and report issues.
• Assist with toileting program activities.
• Complete flow sheets and document progress daily.
• Notify nurse of any factors affecting performance.
