Job summary

Hillcrest HealthCare System is seeking a day shift, full-time Clinical Navigator RN for the Hepatobiliary Clinic in Tulsa, OK. The RN will coordinate patient care across the continuum, act as a clinical resource, and advocate for patients and families to foster high-quality outcomes.

Preferred BSN and cardiology/critical care experience. Must have RN license and BLS within 14 days of hire. Excellent communication and care-management skills are essential for success in this role.

Qualifications

• Graduate of an accredited school of nursing.
• Current Registered Nurse (RN) license.
• BLS certification must be obtained within 14 days of hire or transfer into the role and prior to providing direct patient care.

Responsibilities

• Coordinates and facilitates patient care across the continuum of care for designated patient populations.
• Serves as a clinical resource, patient advocate, and liaison among patients, families, providers, and interdisciplinary teams to promote high-quality outcomes and a positive patient experience.
• Supports care coordination, patient education, clinical program initiatives, and evidence-based practice standards.
• Contributes to improved clinical outcomes through effective communication, collaboration, and patient-centered care management.
