Trinity Health is seeking an Ambulatory Triage Nurse (RN) for day shifts at our Elmhurst and Oak Park locations. The role focuses on patient assessment, triage, and coordinated care in an ambulatory setting, with emphasis on comprehensive, person-centered care and ongoing professional growth.

Qualifications

• Diploma from an accredited school/college of nursing. (BSN or ADN)
• BLS certification required prior to hire
• Current RN license State of Illinois

Responsibilities

• Conducts patient care and assessment including vital signs and history.
• Performs initial Nurse Triage to prioritize care.
• Coordinates care with physicians and specialists for timely treatments.
• Educates patients and families on managing conditions and medications.
• Maintains accurate patient records and schedules appointments.
