Mentor Community Services, part of the Sevita family, seeks a full-time Program Supervisor for the Southside Jacksonville location. The role oversees a day program center, supervising Direct Support Professionals and ensuring delivery of supports per individual plans.

Flexible supervision hours and direct support relieve resident needs while maintaining program compliance. Salary at $19.00 per hour, with 8am–5pm hours and 8–16 hours weekly in supervision; candidate must meet essential functions

Qualifications

• High School diploma or equivalent required.
• At least 1 year related work experience preferred.
• Must be able to supervise Direct Support Professionals and manage daily program operations.

Responsibilities

• Supervise Direct Support Professionals and oversee day-to-day operation of the program.
• Document and review records and billing with accuracy.
• Assist in development and monitoring of individual program plans; attend routine progress meetings.
