Responsibilities include implementing individual service plans, documenting progress, coordinating transportation, and ensuring health, safety, and dignity.

Qualifications

• Must be at least 18 years old.
• Valid driver’s license.
• GED or High School Diploma.
• Compassionate, dependable, and team-oriented.
• Strong communication skills and willingness to learn.

Responsibilities

• Lead structured activities including social, recreational, and community-based programming.
• Support individuals in building independence, social skills, and daily living skills.
• Implement individual service plans and support progress toward personal goals.
• Ensure health, safety, and dignity of individuals during program hours, transportation, and community outings.
• Provide personal care and administer medications as needed (training provided).
• Promote inclusion and positive community engagement.
• Utilize crisis prevention and intervention strategies to support individuals safely.
• Complete required documentation including daily logs and incident reports.
• Transport individuals and supervise activities in community settings.
• Prepare meals and maintain a clean, organized home environment.
