Job summary

Access Services 3 is seeking a Direct Support Professional for our Day Program in Lehighton, PA. The role focuses on providing personalized support to adults with developmental disabilities, implementing ISP and nursing care plans, and promoting independence in daily activities.

Responsibilities include assisting with daily living tasks, documenting progress, and coordinating with staff to ensure a safe, inclusive environment. A valid driver’s license and ability to travel locally are required.

• Ability to provide personal care as needed.
• Ability to establish and maintain effective working relationships.
• Ability to exercise good judgment and discretion.
• Ability to communicate effectively, both orally and in writing.

Responsibilities

• Implement the Individual Support Plan (ISP) and Nursing Care Plan when applicable.
• Support individuals with daily living activities, including personal care and meal preparation.
• Promote independence in daily activities, learning, leisure, and volunteering.
• Transport individuals to and from the program and community sites as needed.
• Maintain accurate documentation of progress, incidents, and observations.
