Job summary

Children’s Wisconsin is hiring a Project Coordinator for Children and Youth with Special Health Care Needs to coordinate grant objectives, provide trainings, and connect families with information and referral services in southeast Wisconsin.

The role acts as a regional consultant, promoting medical home quality improvements, supporting health system partnerships, and leading outreach efforts to engage families and community partners across eight counties in the region.

Qualifications

• 2+ years of related experience in healthcare, public health, social work or nonprofit.
• Experience with collaborative statewide/community efforts.
• Excellent written and verbal communication.
• Ability to build and sustain partnerships.
• Understanding of racial and ethnic disparities in health.
• Knowledge of quality improvement tools/methodologies.
• Familiarity with regional CYSHCN resources in SE Wisconsin.

Responsibilities

• Coordinate work around grant objectives and deliverables.
• Provide trainings, information and referral services, and service coordination to families of CYSHCN.
• Act as a regional consultant to healthcare practices and systems for medical home quality improvement.
• Promote trainings and presentations on medical home and youth health transition for families and partners.
• Support quality improvement grantees focusing on shared plans of care within Wisconsin initiative.
• Recruit and support potential grantees across an eight county region.
• Participate in case review meetings to ensure best practices in service coordination.
