Job summary

Wayne County NY is seeking a qualified professional to support the Children with Special Health Care Needs program within the Public Health Department. You will assist the Supervisor with work plans, policies, contracts, and community/provider relations, while maintaining accurate program records.

The role emphasizes collaboration with public stakeholders, data management, and ongoing policy updates, with emphasis on youth aged 0–21 and transition planning to adult care.

Qualifications

• Bachelor’s degree (or equivalent experience) in a relevant field.
• Experience with the administration, delivery, and monitoring of adolescent services.
• Ability to work with diverse families and knowledge of community resources.

Responsibilities

• Create and implement work plans for CYSHCN program.
• Update policies and procedures yearly or as needed.
• Attend meetings and engage with community providers and schools.
• Complete quarterly program reports and maintain PERDS NYS data.
• Coordinate community resources and referrals; support youth transition to adult care.
