Children's Wisconsin coordinates work around grant objectives and provides trainings, information and referral services to families of children with special health care needs. Acts as a regional consultant to area health care practices and systems aligning with best-practices in service and care coordination.

Supports health system and community initiatives focused on medical home quality improvement and engages diverse partners to promote care coordination across systems.

Formación

• Associate's or Bachelor's degree preferred; healthcare/public health exposure a plus.
• Experience working with families of children with special health care needs preferred.
• Ability to collaborate across hospitals, clinics, and community partners.

Responsabilidades

• Coordinate grant objectives and deliverables for regional programs.
• Provide trainings, information and referral services to families.
• Act as a regional consultant to practices aligning with best practices.
• Support quality improvement initiatives for medical home implementation.
• Engage with community partners to promote care coordination across systems.
• Maintain knowledge of health benefits, waivers, and education resources.
