Job summary

Kaiser Permanente in Lafayette, Colorado is seeking a Certified Wound/Ostomy/Continence Nurse Practitioner (CWOC NP) to manage outpatient caseloads with complex wound and ostomy care needs. The role collaborates with physicians and providers, assesses and plans patient care, and educates staff and families.

Clinical expertise in wound, ostomy, and continence care is required, with licensure in Colorado and credentialing.

Qualifications

• Minimum two (2) years of NP nursing experience in home care, long-term care, hospital or ambulatory care clinic setting.
• Minimum one (1) year of wound and/or ostomy experience.
• Current evidence-based knowledge of wound, ostomy, and continence nursing practice.
• Must meet credentialing requirements established for Nurse Practitioners.

Responsibilities

• Wound, Ostomy, Continence Care: Assess, examine, counsel, and determine plan of care for wound, ostomy, and continence care.
• Collaborate with physicians and other healthcare providers in hospital, clinic, home health/hospice and facilities.
• Provide education of nursing, medical, and other staff regarding wound and ostomy management.
• Participate in quality management/improvement activities in the CO Region.
