Kaiser Permanente in Downey, CA is seeking a Wound Ostomy Continence nurse to provide specialized care and education to patients with ostomies, draining wounds, and related skin issues. This per diem role emphasizes direct patient care and learning opportunities.

You will work with a multidisciplinary team to evaluate products, educate patients, and support continuity of care across the treatment continuum.

Responsabilidades

• Provide follow-up and interventions to patients diagnosed with ostomies, draining wounds, fistulae, or skin breakdown.
• Counsel individuals anticipating surgery that may result in the creation of a stoma.
• Provide individualized instruction for patients with an abdominal stoma for stool or urine diversion.
• Assist patients through the continuum of care for management of related issues.
• Evaluate new and currently used products for ostomy, continence, and wound care.
• Make recommendations to patients for obtaining related products.
• Assess individual patient needs and goals, providing educational programs on ostomy/wound care management.
• Conduct continuing educational programs for physicians and nursing personnel.
