Position Overview

As a Patient Care Coordinator, you will:

1. Coordinate and manage patient care activities through telephonic and electronic communication;
2. Conduct initial and ongoing patient assessments to determine appropriate care and resources needed;
3. Collaborate with healthcare providers to develop and implement individualized care plans;
4. Monitor patient progress and provide support and guidance as needed;
5. Communicate with insurance companies to obtain necessary authorizations for medical procedures and medications;
6. Maintain accurate and up-to-date patient records;
7. Identify and resolve any issues or concerns that may arise during the course of patient care.
