Job summary

CN Guidance & Counseling Services is seeking a Care Coordinator to join the Critical Time Intervention Team. You will deliver time-limited, phase-based care management, develop person-centered plans, and connect clients with community resources to support continuity of care during transitional times.

Responsibilities include assessing clients, developing care plans with the supervisor, coordinating with healthcare providers, and providing outreach in the field.

Qualifications

• Bachelor's or Master's in Social Work, Psychology, Education, Rehabilitation, Counseling or related field preferred.
• Experience providing direct services to individuals with mental health and/or substance use disorders.
• Excellent written and verbal communication skills; ability to document per program standards.
• Knowledge of Medicaid, Social Security and entitlements preferred.

Responsibilities

• Conduct initial and ongoing assessments to document strengths, needs, goals, and resources.
• Develop and document client-centered, interdisciplinary care plans with supervision.
• Maintain communication with clients, healthcare providers, family, and resources.
• Coordinate care planning and transitions with other providers.
