Job summary

Ensemble CIN is seeking a Care Coordinator I to help patients move safely and confidently from one care setting to the next. You will be the steady voice on the phone, ensuring discharge instructions are clear, follow-up appointments occur, and nothing slips through the cracks.

You will contact patients and families via phone, EMR, and remote monitoring, coordinating with clinical staff to address barriers and keep care connected.

Qualifications

• High school diploma or equivalent.
• Scheduling experience preferred.
• One year of experience in a healthcare or customer service setting preferred.
• Medical Assistant certification preferred.
• Bilingual Spanish-English communication skills preferred.
• Comfort with electronic medical records and everyday communication platforms.
• Working knowledge of medical terminology, confidentiality, and HIPAA.
• Strong people skills and the ability to juggle many priorities.

Responsibilities

• Reach out to patients and families by phone, EMR, or remote monitoring to confirm services and address non-clinical barriers to care.
• Ensure patients understand discharge instructions and complete follow-up appointments.
• Connect patients to community resources that support recovery.
• Assist with scheduling and service verification for post-discharge care.
• Monitor remote patient alerts and escalate concerns to clinical staff as needed.
• Document all communications accurately in the EMR.
• Collaborate with physicians, providers, and community agencies to keep care connected.
• Share ideas to improve workflows and patient experience.
