Job summary

Dimock Center is seeking a Per Diem Case Manager for Clinical Stabilization Services (CSS) to provide care coordination and discharge planning within a residential treatment setting. You will collaborate with clinicians, recovery specialists, medical providers, and community partners to help clients access services and transition back into the community.

The role requires a Bachelor’s degree, at least one year in a substance-use treatment program, and strong communication skills.

Qualifications

• Bachelor's degree in Human Services, Psychology, Sociology, Health Services, or related field.
• Master's or MSW enrollment preferred but not required.
• At least one year of experience working in a substance use treatment program.
• Ideal for candidates seeking experience while finishing graduate studies.

Responsibilities

• Meet with clients to provide care coordination and aftercare planning.
• Conduct assessments to identify client needs, strengths, and barriers to treatment goals.
• Develop care plans with clients and treatment team.
• Coordinate referrals to housing, employment, healthcare, and financial supports.
• Coordinate care with courts and primary/behavioral health providers.
• Facilitate psychoeducational groups.
• Provide transportation to appointments.
• Plan discharge and aftercare to ensure continuity of care.
• Conduct post-discharge follow-up.
• Assist clinical team with crisis management.
• Maintain professional boundaries.
• Complete documentation per Joint Commission standards.
• Attend staff meetings and trainings.
