Job summary

The Dimock Center in Boston, MA is seeking a Per Diem Case Manager for the Clinical Stabilization Services. You will provide care coordination, discharge planning, and linkage to housing, employment, healthcare, and supports within a residential substance use treatment setting.

This role requires strong communication, collaboration, and confidentiality, with a schedule Friday to Monday, 7:00am–3:30pm per week. Bachelor’s degree is required; MSW is preferred but not required.

Qualifications

• Bachelor's degree or higher in a related field; MSW preferred but not required.
• Proven ability to coordinate care and support planning for individuals in treatment.
• Experience with substance use treatment and community referrals is advantageous.

Responsibilities

• Meet with clients regularly for care coordination and aftercare planning.
• Conduct assessments to identify needs, strengths, and barriers to treatment.
• Develop care plans with clients and the treatment team.
• Coordinate referrals to housing, employment, healthcare, and financial supports.
• Coordinate care with courts, primary care, and behavioral health providers.
• Facilitate psychoeducational groups as assigned.
• Provide transportation to appointments as needed.
• Plan and coordinate discharge and aftercare to ensure continuity of care.
• Complete timely documentation in line with Joint Commission standards.
