Job summary

Qualifications

Responsibilities

• Assess, plan, implement and evaluate patient care.
• Coordinate care with physicians, peers and support services.
• Provide patient/family education and discharge planning as needed.
• Document care and patient status with electronic records.
• Administer medications using bar-scanning and accurate documentation.
• Support a safe and effective transition of care for patients.
