Job summary

You will participate in multidisciplinary rounds, perform medication order reviews, and provide dosing and drug information expertise to the care team. Required is a PharmD from an ACPE-accredited school, with PGY-2 residency and several years of specialty experience, or equivalent combined credentials.

Qualifications

• Graduated from an ACPE accredited College of Pharmacy (required).
• Post-Graduate Year 2 Specialty Residency plus 3 years of experience in specialty area OR PGY-1 Residency plus 5 years OR 10 years of experience in specialty area.

Responsibilities

• Perform medication order evaluation and verification for appropriateness and efficacy.
• Provide drug information and pharmacokinetic dosing consultations.
• Supervise pharmacy technicians and support staff.
• Participate in formulary management and interdepartmental cost containment.
• Conduct patient counseling and medication histories during rounds and transitions of care.
• Collaborate with physicians and nurses to optimize pharmacotherapy.
• Document clinical interventions and outcomes in the EHR and CDS systems.
• Support education and precept PGY1/PGY2 residents and students.
