Job summary

Vanderbilt Health is seeking a Clinical Pharmacist for the Medical ICU and medicine stepdown to provide comprehensive pharmaceutical care to adult patients. You will participate in rounds, support transitions of care, AMS stewardship and code response, while ensuring safe, cost-effective medication therapy.

The role requires leadership in pharmacotherapy optimization, patient education, and collaboration with multiple disciplines.

Qualifications

• Completion of ASHP-accredited PGY2 residency or equivalent in critical care/medicine.
• ASHP PGY1 residency with critical care experience is preferred.
• Equivalent years of relevant clinical practice in critical care accepted.

Responsibilities

• Reviews drug orders to ensure safe, appropriate and cost-effective use.
• Prepares and dispenses medications and provides drug information.
• Monitors and improves medication therapy safety and outcomes.
• Supervises support staff and ensures accuracy of dispensed medications.
