Job summary

Vanderbilt Health is seeking a Clinical Pharmacist to lead a specialized healthcare team and optimize patient drug therapy within Vanderbilt University Hospital. The role focuses on developing pharmacotherapeutic goals, monitoring plans, and preventing medication errors to enhance patient safety.

The position involves participating in interdisciplinary rounds, providing antimicrobial stewardship, and ensuring high-quality pharmaceutical care for adult critically ill patients.

Qualifications

• Pharmacist license in Tennessee or eligibility to obtain one.
• Bachelor's degree in Pharmacy is required.
• Clinical experience in critical care or acute care settings is preferred.

Responsibilities

• Reviews drug orders and prescriptions to ensure safe, appropriate and cost-effective use of medications.
• Prepares and dispenses medications, provides drug information and manages medication therapy per regulations and hospital policy.
• Maintains quality, safety, and consistency of clinical pharmacy services in assigned areas.
• Supervises activities of support personnel and verifies accuracy of all medications prior to dispensing.
• Additional duties may be assigned as needed.
