Job summary

Community Solutions for Children, Families and Individuals is seeking a Community Support Specialist for its Crisis Residential program. You will work under the Program Manager to assist clients and families, performing screening, intake, assessments, and referrals within Santa Clara County and surrounding areas.

The role requires high school diploma or GED with direct service experience; bilingual abilities and CADC are preferred.

Qualifications

• Requires high school diploma or GED with 4 years direct service work.
• Bachelor’s degree preferred; CADC certification considered.
• Bilingual and bicultural preferred.

Responsibilities

• Screen, intake, and assess clients; develop Transformational Care Plans (TCP).
• Provide rehabilitation counseling, advocacy, crisis intervention, and referrals.
• Complete detailed program documentation and maintain charts.
• Coordinate services with other agencies and supports as needed.
• Attend meetings and participate in quality improvement initiatives.
