Job summary

Community Solutions is seeking a Community Support Specialist to assist clients and families across Santa Clara and San Benito Counties. The role supports program goals under the Bridge to Care Implementation with supervision by the Program Manager.

Responsibilities include screening, intake, assessments, developing Transformational Care Plans, counseling, referrals, documentation, and collaboration with community organizations.

Qualifications

• High School diploma or GED with four years direct service work experience.
• Bachelor’s degree in a related field is preferred.
• Certified Alcohol Drug Counselor (CADC) is preferred.
• Bilingual and bicultural abilities are preferred.

Responsibilities

• Conducts screening, intake and assessments to identify resources and creates Transformational Care Plans with documentation.
• Provides rehabilitation counseling, advocacy, crisis intervention, and referral services using evidence-based practices.
• Completes program documents and maintains charts per agency regulations.
• Coordinates services with other advocacy programs, legal system, and community providers.
• Teaches life skills and assists with educational and support activities for clients and families.
• Maintains excellent customer service with internal and external customers.
• Develops relationships with community organizations to support client needs.
• Requires automobile for client visits and transportation duties.
• Responds to communications within 24 hours and collaborates on problem solving.
