Job summary

Community Solutions for Children, Families, and Individuals is seeking a Community Support Specialist to deliver direct client services across Santa Clara and San Benito counties, under supervision.

You will screen and assess clients, develop Transformational Care Plans, coordinate referrals, provide counseling and crisis support, and maintain detailed documentation, ensuring compliance with contracts and program requirements.

Qualifications

• High School diploma or GED and four (4) years direct service work experience in a related field.
• Bachelor’s degree preferred in a related field.
• Certified Alcohol Drug Counselor (CADC) preferred.
• The individual will preferably be bilingual and bicultural.
• Empowerment, advocacy and support techniques for victims/survivors of violence, people with mental illness or substance abuse issues.
• Working knowledge of wellness and recovery concepts.
• Knowledge of case management, group and individual rehabilitation counseling, and linkage-referral.

Responsibilities

• Conducts screening, intake and assessments; determines resources and implements Transformational Care Plans (TCP); maintains documentation.
• Provides rehabilitation counseling, advocacy, crisis intervention, parenting classes/education and referrals using evidence-based practices and wellness & recovery model.
• Completes detailed program documents per agency, program and regulatory requirements.
• Maintains charts within Agency regulations.
• Coordinates services with other advocacy programs, the legal system, and community providers.
• Teaches life skills and supports educational activities; helps access community services.
• Maintains excellent customer service with internal and external customers.
• Develops and maintains collaborative relationships with community organizations and system providers.
• Operates personal or Agency vehicle for client visits and transportation.
• Responds to calls/emails within 24 hours; resolves issues using strength-based solutions.
• Attends Agency meetings and contributes to quality improvement activities.
• Uses strength-based and solution-focused problem-solving skills.
• Completes training requirements and participates in on-call rotations; may work some evenings/weekends.
• Performs first aid as needed.
