Responsibilities

• Identify and assess caller/client needs.
• Provide crisis intervention and link to community resources.
• Deliver prevention and education services.
• Maintain accurate service documentation in electronic health records.
• Coordinate referrals and determine appropriate level of care.
• Develop and implement intervention goals and plans.
• Track and evaluate program data for quality and fidelity.
• Adhere to professional standards and regulations.
