Job summary

First Choice Services is seeking a SUD Peer Crisis Case Manager in West Virginia to provide follow-up for individuals interacting with the 988 Crisis System, promoting engagement and ongoing care after crises.

You will coordinate with MCOTs, assess risk, and tailor recovery‑oriented interventions for diverse populations, including LGBTQI+ youth and rural residents, while upholding privacy and ethical standards.

Qualifications

• High School Diploma or GED required; four‑year degree in social/behavioral health preferred.
• One year of crisis intervention experience.
• Excellent written and verbal communication skills.
• Compassion and empathy to respond to client needs without bias.
• Ability to document on data systems while assisting via call, text, or chat.
• Emotional fortitude to handle difficult client situations in a fast-paced environment.
• Respect for privacy rights under HIPAA and related laws.

Responsibilities

• Provide follow-up services to consenting consumers of the 988 Crisis System.
• Implement a follow-up protocol within 24 hours of referral or care transition.
• Assess suicide and overdose risk for up to 12 months as needed.
• Coordinate with Mobile Crisis Outreach Teams to promote de-escalation and continuity of care.
• Tailor interventions to client and support networks to support recovery.
• Deliver services designed to support substance use recovery through peer principles.
• Maintain knowledge of state services, supports, and treatment options.
• Build relationships with crisis stabilization resources across the state.
