Job summary

The Community Care of West Virginia Inc. is seeking a Case Manager to oversee an assigned caseload of clients with addiction-related illnesses in Bridgeport, WV.

You will assess needs, develop and implement treatment plans, and coordinate services with the care team to support recovery. Responsibilities include documenting contacts, arranging referrals, and maintaining up-to-date patient records while complying with regulatory standards and CCWV policies.

Qualifications

• Bachelor's degree required in a related field.
• Experience in behavioral health or substance use treatment preferred.
• Knowledge of assessments and treatment planning practices.
• Ability to document and communicate effectively across teams.

Responsibilities

• Assess needs and develop treatment plans for caseload.
• Coordinate services and referrals with treatment providers and community resources.
• Maintain accurate and timely documentation in patient records.
• Communicate with patients and families with empathy and respect.
• Attend interdisciplinary staff meetings and participate in care planning.
