Qualifications

The Crisis Outreach Care Manager’s main purpose is to facilitate enrollment and discharge, conduct follow ups, community outreach, and provide crisis intervention for all age groups. Responsible for the delivery of care management, advocacy and clinical services to the Seriously Mentally Ill and the Mental Health participants. This position shall intervene to alleviate the participant’s immediate social, emotional or environmental crisis and assess, evaluate, triage and refer the participant for appropriate follow-up treatment to support a return to an effective lifestyle. The Crisis Outreach Care Manager will respond to any requested crisis call in person within 30 minutes or by telephone within 15 minutes while ensuring meeting emergency demands in a calm, competent, productive and timely manner. This position will require an individual to spend at least 50% of time out of the office meeting with community members and stakeholders including face to face in the community and people's homes and educate them about entitlements and Behavioral health Services; educate and inform community members and organizations about services; and provide consultation education and advocacy services to families and other providers.

Qualifications Required
  • High School Diploma or GED plus 4 years’ experience in behavioral health or combined behavioral healtheducation and experience to meet qualifications for Behavioral Health Technician (BHT) qualification.
  • Must be at least 21 years of age or older.
  • Possess a valid driver's license with acceptable driving record and availability by telephone.
  • First Aid, CPR and CPI training certification (agency will provide training if do not possesscurrent certification).
  • Able to work flexible schedule as needed: weekends, nights, days, evenings, and holidays.
  • This position requires successful completion of background check along with obtaining and maintaining a Level 1 Fingerprint Clearance card through Arizona DPS and clearance with the Pinal County Sheriff’s office to ensure access to the Correctional Facilities.
  • Knowledge of local communities, resources, and entitlements for various funding sources.
  • Trained in crisis intervention and offered services to agency participants and residents in the community.
  • Able to interact effectively with a variety of individuals and groups from varying education, ethnic and socio-economic backgrounds to include listening, sensitivity to the other feelings, needs and point of view, tact, and courtesy in expressing opinions or ideas, projecting a positive image of the Agency and behavioral health services.
  • Prior experience in the behavioral health setting including extensive knowledge and background of SMI population, and substance abuse.
  • Knowledge of how the acute and chronic nature of the chemical /co-dependence and the ways medical, emotional, behavioral, social, educational, and other interventions are used to treat the participant.
  • Demonstrable theoretical knowledge in the field
  • Must be knowledgeable of benefit and entitlements process and community resources available.
  • Must be familiar with current psychotropic medications and side effects.
  • Must have good oral and written communication skills.
  • Ability to develop treatment plan and summary based on participants needs.
  • Ability to understand the bio psychosocial influences and effects of chemical/co-dependencies.
  • Ability to type and working knowledge/familiarity with computers and electronic clinical record keeping.
Mental and Emotional
  • Proven ability prioritizing workload, decision-making ability, using sound judgment to field inquiries concerning administrative, clinical and operational concerns and ability to greet and deal effectively with participants, families, staff and community members.
  • Able to positively interact and develop rapport with participants and their families, professional and support staff and various levels of staff from community agencies.
  • Appropriately interact with participants without regard to gender and cultural variables.
  • Able to maintain a calm, non-defensive, supportive attitude during crisis or potential crisis situations.
  • Constant problem solving and reading/review analysis.
  • Ability to analyze variables and situations.
  • Ability to resolve problems, handle conflict and make effective decisions under pressure.
Physical
  • Physical requirements include long periods of sitting and ability to bed, left and carry up to 50 pounds.
  • Frequent walking, sitting and occasionally standing. Must be fully ambulatory and able to assist participants.
  • Able to operate phone, Fax, copier, and a computer.
  • Able to operate Agency vehicle.
  • Encounter occasional hazards such as but not limited to bodily fluids and potentially infectious materials and occasional breathing in odors, dust, smoke, and gases.
  • Must be able to operate an Agency vehicle.
Qualifications Preferred
  • Bilingual English/Spanish
  • Knowledge of area and services to special populations including experience with seriously mentally ill participant population.
  • Bachelor’s Degree in human service or related field.
  • Advanced knowledge of cultural diversity and sensitivity
