• Valid driver’s license, auto insurance, and a reliable vehicle for work use.
• 2+ years of experience in substance use or behavioral health.
• Excellent written, verbal, and interpersonal communication; proficient with MS Office.

Responsibilities

• Provide compassionate, person-centered support in an outpatient urgent care setting.
• Engage clients during crisis visits to promote stabilization and treatment participation.
• Build trusting relationships using trauma-informed, recovery-oriented approaches.
• Connect clients with housing, transportation, financial assistance, healthcare, employment and other resources.
• Coordinate follow-up care and transport to appointments using your vehicle as needed.
• Document client interactions, referrals, progress, and services in the Carelogic EHR.
• Collaborate with therapists, nurses, prescribers, peer support specialists, and the team for coordinated care.
• Monitor client progress post-crisis and communicate changes to the treatment team.
