Job summary

ReDiscover is seeking a Community Support Specialist to join its CPRC in Lee's Summit, MO. You’ll maintain client caseloads, support symptom management, employment, and independent living outcomes, and collaborate with a multidisciplinary team to coordinate care.

Requirements include a bachelor’s degree (or 4 years of equivalent experience), valid driver’s license with a reliable vehicle, DBT experience, and strong communication and customer service skills.

Qualifications

• 4 year degree or 4 years of equivalent experience in lieu of degree.
• A valid driver’s license, appropriate auto insurance and a safe, reliable vehicle for use during work hours including transporting clients in personal vehicle.
• Willingness to work with Dialectical Behavior Therapy (DBT).
• At least two years clinical experience in direct service provision to children and adolescents in a mental health setting preferred.
• Knowledge of Kansas City metro community resources and health/social service systems; ability to work with community partners.
• Excellent customer service skills with a recovery focus.
• Strong written, oral and interpersonal communication skills.
• Intermediate computer skills: typing, Microsoft Word and Excel, and email.

Responsibilities

• Maintain and interact with a caseload of clients in-person or by phone.
• Facilitate each client’s achievement of long-range outcomes including symptom management, employment and independent living.
• Engage difficult-to-serve clients by identifying resources to gain access and connections.
• Help remove barriers by linking clients to community or social service programs and referrals to education or work programs.
• Assist clients with transportation barriers as identified in the treatment plan.
• Collect and maintain information about clients and document interactions in the electronic medical record.
• Communicate and collaborate with an internal multi-disciplinary team to coordinate client care.
