Job summary

ReDiscover is seeking a Community Support Specialist to join the CPRC Program in Kansas City. You will maintain a caseload, support independence, and help clients access services.

This role involves coordinating transportation, collaborating with a multidisciplinary team, and guiding clients toward employment and living goals. The position values compassionate, client-centered care and offers growth through training and meaningful work.

Qualifications

• 4 year degree or 4 years of equivalent experience in lieu of degree.
• A valid driver's license, appropriate auto insurance and a safe, reliable vehicle for transporting clients.
• At least two years clinical experience in direct service provision to children/adolescents preferred.
• Knowledge of KC metro resources and health/social service systems; building community partnerships preferred.
• Excellent customer service with a recovery focus.
• Strong written, oral and interpersonal communication skills.
• Intermediate computer skills: typing, Word, Excel, and email.

Responsibilities

• Maintain and interact with a caseload of clients in-person or over the phone.
• Facilitate clients' achievement of long range outcomes in symptom management, employment and independent living.
• Engage difficult-to-serve clients by investigating resources to gain access and connections.
• Remove barriers by connecting clients to community or social programs and referrals.
• Assist clients with transportation barriers as identified in treatment plans.
• Collect and maintain client information and document interactions in the EMR.
• Collaborate with a multidisciplinary team to coordinate client care and wellness.
