Socket.dev in Chicago, IL is seeking a Crisis Continuum of Care Intake Specialist to serve as a first point of contact for individuals and partners seeking behavioral health crisis services. This on-site role coordinates referrals, completes intake screenings, and maintains accurate documentation across MRSS, CSP, and 590 programs.

Qualifications

• High School Diploma or equivalent.
• One year of experience in a related field.
• Experience with Illinois Medicaid programs or DHS/IDHS is preferred.
• Familiarity with electronic health record systems.

Responsibilities

• Serve as the primary point of contact for crisis referrals.
• Answer 988 referral line and coordinate incoming MRSS referrals.
• Complete intake screenings and gather demographic and clinical information.
• Coordinate referrals to Crisis Continuum of Care services and staff.
• Maintain accurate documentation in the electronic health record.
• Monitor call volume and prioritize referrals.
• Collaborate with hospitals, schools, and community partners.
