Job summary

Housing Forward is seeking a dedicated Specialist to serve as the front door to the agency’s homelessness crisis-response. You will perform in-person assessments for walk-ins to determine immediate housing and stabilization needs and connect clients with case management, referrals, and financial or employment assistance.

The role includes crisis-diversion duties, coordinating with multiple Housing Forward services, and maintaining HMIS/Salesforce records with timely notes.

Qualifications

  • Bachelor’s degree in social work or related field.
  • 2-5 years of experience working with individuals in crisis or homelessness.
  • Proficiency in Microsoft Office (Word; Excel) required.
  • Proficiency with computers and VOIP phone systems.
  • Must have smartphone to utilize Agency communication and collaboration tools.

Responsibilities

  • Perform screenings to determine appropriate response to imminent housing crisis.
  • Determine underlying issues and provide solutions to resolve crisis.
  • Maintain a small caseload and conduct sessions as needed.
  • Maintain knowledge of housing options, budgets, and employment programs.
  • Coordinate interventions with Housing Forward services.
  • Greet and screen walk-in clients; refer to HP or Homeless Programs as appropriate.
  • Track referrals, shelter turnaways, and enter assessments into HMIS and Salesforce.
  • Attend staff meetings and represent the organization professionally.
