Job summary

Highland Rivers Behavioral Health in Cedartown, GA is seeking a Case Manager to provide direct care to adults in crisis. You will assist in discharge planning, link individuals to aftercare services, and support the treatment team in coordinating transition plans.

Requirements include a 4-year degree, 2 years' behavioral health experience, bilingual English/Spanish preferred, and completion of background checks and screenings.

Qualifications

• Bachelor’s degree in Psychology, Social Work, Sociology, Human Services, Criminology, or a related field.
• 2 years of professional experience in behavioral health, specifically focusing on a psychiatric setting.
• Criminal background check clearance, official transcripts, valid driver’s license/ID, drug screen, TB test, and eligible 7-year MVR.

Responsibilities

• Assist individuals with identifying aftercare needs and developing a discharge plan through interviews with the individual and family when appropriate.
• Participate in treatment team to assist with discharge planning needs.
• Provide linkage to aftercare services by arranging appointments, transportation and supports.
• Provide follow up to improve engagement in aftercare services and reduce recidivism.
• Monitor the individual’s wellbeing and report concerns to supervisor.
• Maintain clinical records and documentation according to agency policies.
• Assist in admission and discharge processes and orient new individuals to the unit.
