Job summary

Universal Health Services seeks a Needs Assessment and Referral Counselor to provide telephonic and in-person risk assessment and crisis intervention. You will contact referral sources and potential patients, coordinate referral and intake, determine level of care, and assist with admission while maintaining communication with families, patients, and unit staff.

Role requires experience in psychiatric settings, strong communication skills, and the ability to navigate complex patient referrals

Qualifications

• Licensed professionals required (e.g., Social Work, Counseling, Psychology, Nursing).
• Experience with intake, admission and crisis intervention in psychiatric settings preferred.
• Ability to coordinate referrals and communicate with families, patients and staff.

Responsibilities

• Provide telephonic and in-person risk assessment and crisis intervention.
• Make initial contact with referral sources and potential patients.
• Coordinate referral and intake process and determine appropriate level of care.
• Facilitate admission process and maintain ongoing communication with referral sources, families, patients and unit staff.
