Job summary

Jobtailor is seeking a compassionate professional to provide intake and assessment for individuals and families experiencing homelessness. You will interview clients, prioritize needs, and link with other agencies, while maintaining confidentiality and thorough HMIS documentation.

This role requires a High School diploma, weekend availability, and training in crisis response; Inform USA certification is encouraged.

Qualifications

• High School diploma is required.
• Experience with human services preferred.
• Experience responding to people in crisis is desired.
• Completes initial and ongoing trainings (including CEUs).
• Inform USA certification is a plus; weekend availability is required.

Responsibilities

• Provide intake and assessment by phone and electronically.
• Interview clients and prioritize needs based on resources.
• Facilitate linkage with other agencies to assist clients.
• Advocate for clients facing barriers to services.
• Respond to crisis or emergencies with appropriate actions.
• Follow up to ensure needs are met and address endangerment cases.
• Maintain confidential documentation in HMIS and other systems.
