Job summary

Connections Health Solutions in San Francisco’s RESET Center seeks a Case Manager to coordinate care, engage clients, and plan discharges for individuals in the sobering program. You will stabilize participants, assess needs, and connect them to ongoing community resources.

Responsibilities include screenings, crisis de-escalation, and close collaboration with local law enforcement, emergency departments, and outpatient providers.

Qualifications

• 1-3 years of experience in harm reduction, crisis services, behavioral health, or related community care settings.
• High School diploma or equivalent.
• Experience supporting law enforcement diversion or working alongside first responders, emergency departments, or criminal-legal partners.
• Must obtain Overdose Prevention and Narcan Administration training within 30 days of hire.
• Basic computer literacy, including documenting in an electronic health record (EHR).
• Ability to obtain and record vital signs.
• Strong attention to detail and timely documentation.
• Must complete Basic Life Support (BLS) certification in first week.
• Company vaccination policy requires full vaccination or approved exemption.

Responsibilities

• Welcome individuals with empathy, respect, and a non-judgmental approach in a low-barrier, trauma-informed environment.
• Engage participants using motivational interviewing to identify needs, goals, and readiness for services.
• Conduct brief screenings and assessments to understand risk, safety, and resource needs.
• Provide education, encouragement, and practical support tailored to participant goals.
• Develop short-term, accessible discharge plans for 4–24 hour stays.
• Coordinate referrals and warm handoffs to community resources (SUD treatment, housing, MH providers).
• Support crisis de-escalation and conflict intervention with the care team.
• Document timely and accurate service entries in Epic.
• Maintain collaborative relationships with law enforcement, public health partners, and community agencies.
• Complete RESET Center trainings.
• Perform all other duties as assigned.
