Job summary

Connections Health Solutions seeks a Case Manager to coordinate care for youth and adults in crisis, guiding admission, planning, and discharge in collaboration with an interdisciplinary treatment team.

The role emphasizes documenting, planning, and coordinating with families and community resources, with background checks and PA resources knowledge. A bachelor's degree or 4 years of experience is required; a Master’s is a plus.

Qualifications

• Bachelor’s degree in behavioral health or related field or 4 years of behavioral health experience.
• Must pass the following background checks: Child Abuse Clearance, FBI, PA State Police.
• Knowledge of Pennsylvania behavioral health system and resources.
• Experience with youth and adults with behavioral health issues.
• Ability to perform Safe Clinch restraint techniques.

Responsibilities

• Facilitate admission intakes and informed consent processes.
• Develop individualized, person-centered Treatment Plans with families where applicable.
• Coordinate care and referrals across facility and external providers.
• Document all services, interventions, and discharge planning.
• Engage with families, schools (youth), and community partners.
• Maintain knowledge of 302 involuntary processes and resources.
