Job summary

Connections Health Solutions in Kirkland, WA seeks a Case Manager III to coordinate admissions, treatment planning, and discharge planning for individuals in crisis. You will work with an interdisciplinary team to deliver personalized care and ensure seamless transitions to community resources.

The role requires a master’s degree in behavioral health, an unrestricted WA LMHP license, and at least 3 years of relevant experience.

Qualifications

• Master’s degree in behavioral health or related field.
• Unrestricted WA LMHP LICSW/LMHC/LMFT or equivalent license required.
• At least 3 years of mental health or substance abuse experience.
• Knowledge of Washington State behavioral health system and resources.
• Vaccination policy compliance and credentialing requirements must be met.

Responsibilities

• Conduct psychosocial assessments in a timely, thorough, and accurate manner.
• Review and sign off on psychosocial assessments by licensed staff.
• Educate individuals on treatment consents and disclosures.
• Participate in Interdisciplinary Treatment Team meetings.
• Develop and implement individualized, person-centered treatment plans.
• Gather collateral to assess treatment and discharge needs.
• Coordinate care and referrals with internal staff and external providers.
• Support safety and discharge planning; maintain accurate documentation.
• Prepare clinical reports and individualized service plans in the electronic system.
