Job summary

Connections Health Solutions in Leesburg, VA seeks a Case Manager to facilitate admission intakes, treatment planning, discharge planning, and personalized care for individuals in crisis. You will work with an interdisciplinary team to coordinate care for patients and families, and maintain accurate EHR records.

A QMHP credential and knowledge of Virginia's behavioral health system are preferred. The role offers shift differentials and comprehensive benefits as part of a full-time roster.

Qualifications

• Bachelor’s degree in behavioral health or related field.
• Knowledge of the Commonwealth of Virginia behavioral health system of care and resources within it.
• QMHP certification.
• Company policy requires vaccination with documentation or approved exemption.
• Virginia Fingerprint Clearance is required.
• Must pass VA DBHDS background check and VA DSS Central Registry Search.

Responsibilities

• Educates and informs individuals of consents for treatment and authorizations for use and disclosures.
• Participates regularly in Interdisciplinary Treatment Team meetings.
• Collaborates with the Treatment Team to develop and implement an individualized treatment plan.
• Gather collateral to assess treatment and discharge needs.
• Coordinate care and referrals with facility staff and external providers.
• Identify gaps and recommend services for discharge planning.
• Engage with the interdisciplinary team to monitor and communicate needs and safety.
• Maintain clinic records and ensure proper signatures and EHR documentation.
• Know and apply Virginia involuntary commitment processes and related laws.
• Participate in supervision, ongoing education, and competency activities.
• Complete required training and attend monthly meetings.

Education

Bachelor’s degree in behavioral health or related field
