Job summary

MIND 24-7 in Phoenix, AZ is seeking a Crisis Case Manager to coordinate discharge planning and ongoing care for patients under psychiatric medication management. The role supports follow-up appointments and access to prescriptions, transportation, insurance, and social services.

You will collaborate with pharmacists, families, and care teams in a fast-paced, 24/7 clinical environment, documenting activities in the EHR and contributing to holistic care planning.

Qualifications

• Associate’s or Bachelor’s degree in healthcare, behavioral health, human services, or related field.
• 1–2 years of experience in case management, care coordination, or behavioral health services.
• Level 1 Fingerprint Clearance Card (or ability to obtain upon hire).
• Strong communication, time management, and organizational skills.
• Familiarity with EHR systems and HIPAA compliance.
• Bilingual (English/Spanish) is a plus.

Responsibilities

What We’re Looking For
