Job summary

NorthKey Community Care is seeking a Crisis Care Coordinator to join our team in Covington, KY. The role focuses on assisting individuals during mental health crises and coordinating care to prevent hospitalizations. Typical hours are Monday–Friday, 8:30 a.m.

Qualifications

• Bachelor’s Degree in an acceptable Behavioral Health field.
• At least one year of experience in the behavioral health field preferred.
• Good written and verbal communication skills

Responsibilities

• Promote environmental and psychosocial health and maximize independence to improve and maintain lifelong well-being and functional status.
• Provide education to clients to promote improved functioning and skills development.
• Assist with crisis response, including triage calls, assessing risk, safety planning, and coordination of appropriate services.
• Coordinate care during transitions between hospitals, correctional facilities, residential settings, and community-based services
• Maintain HIPAA compliance and client confidentiality.
