Job summary

Endeavor Health is seeking a BH Crisis Case Manager to conduct crisis assessments and coordinate treatment planning for patients at Edward Hospital and Elmhurst Hospital in the LOH location.

You will perform comprehensive assessments, document outcomes in Epic, coordinate with multidisciplinary teams, and ensure compliance with EMTALA, Illinois Mental Health Code, HIPAA, and related regulations. Full-time, 36 hours/week.

Qualifications

• Master’s degree or higher in behavioral health is required.
• Strong communication skills are required for difficult situations.
• Knowledge of DSM-5 and ICD-10 diagnostic criteria is required.
• Minimum of two years of assessment experience is required.

Responsibilities

• Conduct crisis clinical assessments for patients at Edward Hospital and Elmhurst Hospital.
• Assess clinical needs and confirm initial diagnostic impression during disposition with physicians.
• Review prior documentation and perform case formulation.
• Provide treatment referrals aligned with medical and psychiatric status.
