Job summary

Endeavor Health is seeking a BH Crisis Case Manager to perform crisis assessments for patients at Edward Hospital and Elmhurst Hospital. You will assess needs, document diagnoses and coordinate referrals with physicians or designees.

Responsibilities include evaluating vital signs, BMI, reviewing records, and collaborating with a multidisciplinary team on discharge planning while ensuring policy compliance and patient rights protection.

Qualifications

• Master's degree or higher in a behavioral health field required.
• Strong communication skills for difficult situations.
• Knowledge of DSM5, ICD-10, and interqual/MCG standards.
• Minimum of two years of assessment experience.

Responsibilities

• Conduct crisis clinical assessments for patients at Edward and Elmhurst Hospitals.
• Assess clinical needs and determine initial diagnostic impressions during disposition.
• Evaluate patients including vital signs, BMI, and related measurements.
• Review previous documentation and form case formulations.
• Provide treatment referrals aligned with medical/psychiatric status.
• Identify problems affecting patients and report promptly.
• Coordinate follow-up with referral sources.
• Obtain collateral information via interviews or calls.
• Consult with multidisciplinary teams on assessment, treatment, and discharge planning.
• Complete admission paperwork and patient rights documentation.
• Coordinate with families and community agencies as appropriate.
• Participate in care conferences and ED huddles.
• Understand hospital policies (EMTALA, Illinois Mental Health Code, HIPAA).
• Understand Behavioral Health reimbursement systems and managed care.
• Obtain pre-certification with insurance when needed.
• Document certification info in Epic and hand off to UM.
• Assist bed management planning with LOH staff.
• Serve as backup for Assessment Specialists when needed.
• Educate patients and families about illness effects on daily life.
