Endeavor Health is seeking a Behavioral Health Clinical Case Manager II at Evanston Hospital. This role provides comprehensive psychosocial assessments, treatment planning, and discharge coordination for inpatients, managing a full caseload of moderate to high complexity.

You will collaborate with an interdisciplinary team, mentor staff, and contribute to quality improvement while supporting transitions of care across settings.

Qualifications

• Master's degree in social work or a recognized mental health field.
• Illinois licensure with LCSW/LCPC/LMFT or Clinical Psychologist required.
• CPR certification within 90 days of hire.
• CPI (Crisis Prevention Intervention) certification within 90 days of hire.
• Minimum of 2 years of experience in a behavioral health environment.

Responsabilités

• Conduct psychosocial assessments and independently develop, implement, and adjust treatment and discharge plans.
• Manage a full caseload of moderate to high complexity patients with co-occurring disorders and discharge barriers.
• Provide crisis intervention and evidence-based therapy across individual, group, and family settings.
• Lead discharge planning and coordinate transitions of care across inpatient, outpatient, and community services.
• Mentor staff, contribute to quality improvement, and collaborate with community providers.
