Job summary

Bridgewell seeks a part-time behavioral support clinician to oversee crisis planning and personal support plans for individuals served through DDS. You will triage emergency calls, coordinate with clinical teams, and contribute to documentation and hospital liaison activities.

Expected duties include in-home training, developing behavior support plans, and on-call crisis response. Requires a BA in human services and 3 years of related experience.

Qualifications

• BA in human services field and 3 years of experience working with dually diagnosed individuals (ID/DD and psychiatric diagnosis) or relevant experience.
• Strong leadership and problem solving abilities.
• Excellent written and oral communication skills.

Responsibilities

• Develop personal support plans outlining crisis management strategies.
• Coordinate crisis planning with team members and service providers.
• Identify preventive resources and services as needed.
• Participate in clinical and triage meetings; support documentation.
• Provide in-home crisis support, training, and education to providers.
• Consult with Sovner/START-affiliated consultants for clients on caseload.
• Maintain updated client records and contact documentation.
• Assist with crisis response, triage, and admission/discharge activities.
• Undertake other tasks designated by supervisor.
