Job summary

Mental Health Cooperative in Nashville, TN is hiring a Care Manager I to provide case management for consumers with serious mental illness within the Criminal Justice Liaison program. You will coordinate with service providers, identify resources, and assist with referrals to support successful community reintegration.

Responsibilities include documentation in TNWITS, caregiver consultation, and participation in team meetings.

Qualifications

• Bachelor’s Degree in a health-related field of counseling, psychology, social work or other behavioral health field.
• Candidates with a bachelor’s degree in criminal justice must have fifteen college level semester hours of coursework in behavioral health.

Responsibilities

• Proactively identify and engage consumers who are seriously mentally ill (SMI) in the Criminal Justice System.
• Identify service providers in organizations serving SMI consumers.
• Develop community resources and coordinate existing resources for this population.
• Meet with identified service providers to educate them about the Organization’s programs and solicit their assistance.
• Monitor progress of consumers’ legal cases and follow up to ensure resources meet needs.
• Assist with referral and linkage to community resources and advocate for consumers.
• Conduct follow-up with consumers after community integration.
• Complete paperwork including TNWITS data entry.
• Provide consultation to other care managers on assessments as needed.
