Mental Health Cooperative, Inc. in Nashville, TN is seeking a Care Manager I to provide case management for the Criminal Justice Liaison program. The role supports consumers with serious mental illness and coordinates with multiple providers. Base pay is $41,000 with additional incentives for language skills and behavioral health experience.

Strong coordination and documentation will be essential, with mileage reimbursement available and adherence to program requirements.

Formación

• Bachelor’s degree in health-related field such as counseling, psychology, social work or related behavioral health field.
• Criminal justice degree holders must have 15 college-level hours in behavioral health.

Responsabilidades

• Identify and engage consumers who are seriously mentally ill in the Criminal Justice System.
• Develop community resources and coordinate existing resources for this population.
• Meet with identified service providers to educate them about the Organization’s programs.
• Monitor progress of consumers’ legal cases and conduct follow-up to ensure resources meet needs.
• Assist with referrals and linkage to community resources and advocate for consumers.
