Job summary

Bluebonnet Trails Community Services is seeking a Court-Based Care Coordinator based at the Williamson County Courthouse. This role focuses on expediting care, coordinating community-based services for individuals ordered into treatment, and collaborating with the Williamson County District Court Administration.

The successful candidate will apply knowledge of mental health, crisis services, and interagency collaboration to provide trauma-informed, person-centered care while balancing

Qualifications

• Bachelor's degree in a human services field.
• Comprehensive knowledge of mental health diagnoses.
• 1 year of experience in community health care.
• 6 months of behavioral health care coordination.
• 3 months of experience with justice-based population.
• Strong written and verbal communication; proficient in Word and Excel.

Responsibilities

• Coordinate care for court-ordered individuals and treatment plans.
• Collaborate with judges, attorneys, jail-based care coordinators and jail diversions.
• Promote trauma-informed, person-centered services to justice-involved individuals.
• Work independently and as part of a team to balance caseloads.
• Maintain thorough documentation and advocate for clients.
